Referrals

My Cancer Care do not require a referral for you to access support. However if you are a health professional and would like to refer a patient, please email us a referral letter.

My Cancer Care take most consults on the spot. All you need to do is call us!

Please make sure you have your diagnosis and treatment plan available as this information is important in accessing your care. You will also be sent a link to a patient intake form to ensure that we have all the details that we need for safe care.

If you would like to book an appointment for education or management then please follow the link above to our booking platform.

My Cancer Care uses a program called Halaxy as our software management. This is where all information is stored and where you can book online appointments.

You will need to fill in a new patient intake form to be able to book online.
